How to Fill In a US Address Form, Field by Field
2026-09-03 · US Address Generator
Whether you are ordering from a US store or registering for a US service, most address-form errors come down to field mapping: Line 1 and Line 2 swapped, a city that does not match the state, a ZIP that belongs somewhere else. Here is each field, one by one.
Field-by-field reference
| Form field | Meaning | Example |
|---|---|---|
| First Name / Last Name | Given name / family name | Marilyn / Monroe |
| Address Line 1 | House number + street | 1234 Oak Street |
| Address Line 2 | Apartment, unit, floor (optional) | Apt 5 |
| City | City | Los Angeles |
| State | State (abbreviation) | CA |
| ZIP Code | 5-digit ZIP | 90001 |
| Phone | Phone with area code | (213) 555-0123 |
Line 1 versus Line 2
Line 1 holds the house number and street name only. Line 2 is for apartments, units, suites and floors. The most common mistake is stuffing "Apt 5" into Line 1 — plenty of address validators will flag it immediately.
Addresses in Chinese run from large to small (country, city, street); US addresses run the opposite way, small to large. Mapping one onto the other means reversing the order as you fill the fields.

Marilyn Monroe 1234 Oak Street Apt 5 Los Angeles, CA 90001 (213) 555-0123
Common errors and fixes
- ZIP does not match the state: prefixes are bound to states; verify both.
- City spelling: forms expect Los Angeles, not LA.
- Missing area code: a 7-digit number will usually fail validation.
- State dropdown: pick the two-letter abbreviation (CA, NY), never a typed full name.
Forwarding warehouses
When you use a package-forwarding service, copy the warehouse address into Line 1 exactly as given and put your member ID into Line 2 — the warehouse uses it to sort your parcels. City, State and ZIP follow the warehouse information, not your own invention.
